Step 2: Fill out forms
To save time on your first visit, you may fill out the necessary paperwork beforehand and bring it with you on your visit. If you plan on using health insurance, please provide us with your information, and we will verify your coverage before your arrival. You may find the forms below or on the Forms Tab.
